Request MyChart Code

To access Vidant MyChart you must obtain an activation code from your current provider/health care team. If you have received your code, you are ready to login to Vidant MyChart. If you have not received an activation code, you may request one by completing this form.

All required information must be entered EXACTLY as indicated and match the information on file in order for us to process your request. For tips on finding your "Clinic Number or Account Number", click here Vidant Bill.

After verifying your information, within 3-5 business days, you will receive an email containing your MyChart activation code, provided the email you listed matches what we have on file. If the email submitted does not match our existing records, your MyChart activation code will be mailed to your home address. If we have any questions about your request or are not able to validate your information, we will contact you at the email address provided.

This form may only be used to request an account for you. To protect the privacy of patient health information, we do not allow individuals to access a family member's or other individual's health record without authorization. Parents, guardians, spouses, adult children, or partners who need access to another account may request Proxy Access from their healthcare team.
